Sound Physicians' ACO Secures $22.9 Million Savings in Medicare Program
Sound Long-Term Care Management (SLTCM), part of Sound Physicians, achieved $22.9 million in shared savings in its first year participating in the Medicare Shared Savings Program. This accomplishment positions SLTCM among the top 10 percent of ACOs nationwide, focusing on long-term care and assisted living patients, traditionally one of the most at-risk demographics.
SLTCM reduced emergency department visits by 8%, admissions by 9.4%, and short-term hospitalizations by 10.9%. Despite their higher cost per beneficiary, SLTCM ranks fifth nationally for per beneficiary per year savings. Their model, emphasizing clinical expertise and partnerships, demonstrates a significant impact on patient care quality and financial performance.
SLTCM serves patients battling severe diseases at a significantly higher rate than average Medicare beneficiaries. Through a collaborative, data-driven approach, SLTCM has transformed care and achieved notable results in patient outcomes and cost savings.
